Membership Terms:

The West Health Spa VIP Membership Program (ESSENTIAL, ELITE and LUX Tiers) is a 12-month membership agreement that may be renewed at the end of the agreement.  All services/treatments, perks, and discounts are subject to change at any time.

Payments

The Membership price will be charged to the customer card on file on the 1st of each month. Essential members pay $115/mo; Elite members pay $230/mo; Lux members pay $420/mo. All members are required to keep a valid credit card on file at all times. The use of gift cards or certificates cannot be used to pay membership fees.

Any membership fee that is returned due to an incorrect card number, misinformation, or insufficient funds is considered to be “behind in payment.” If the balance is not paid in full within 30 days of failed payment, the member is subject to a $50 late fee.

If an outstanding payment is not paid in full on the first of the month, membership services and discounts will not be available for use until the balance is paid in full up to the current month’s date.

Services

ESSENTIAL VIP Members receive one complimentary 60-minute ESSENTIAL VIP treatment per calendar month.

ELITE VIP Members receive two complimentary 60-minute ELITE VIP treatments per calendar month.

LUX VIP Members receive two complimentary 60/90-minute LUX VIP treatment per calendar month.

Services can be rolled over to a future month if they are not redeemed in the month they are purchased. Unused services that roll forward must be used within the current membership year or they will be forfeited. All services that roll forward will expire upon the annual membership renewal date. Services are non-transferable to another person. A valid ID is required to be kept on file at all times to verify the member’s identity.

For all member services, a cancellation call is required at least 24 hours in advance or your service for the month will be forfeited.

PERKS:

All $129 VIP Membership Referral Gift Cards are to give to friends and family.

Perks are not to be combined with any other offer and are not valid for any VIP Membership Member.

Perks are to be used on all SPA services only and do not include any healthcare services such as chiropractic care or functional medicine / functional nutrition / lifestyle nutrition.

Renewal and Cancellation

VIP Memberships automatically renew every 12 months on the anniversary of the purchase date for an additional 12-month term.

Memberships can be cancelled after 6 months of each 12-month term with a 30-day written notice.

Memberships cancelled before 6 months of each 12-month term are subject to a cancellation fee of $200.

Upon entering the contract, you agree to all terms and conditions above.
